PA State Funded Programs
This state funded program offers personal care attendants, payroll for your attendant and supports coordination. Similar to the Attendant Care Waiver program, you can access services if you are an adult with a physical disability. This program differs from the Attendant Care Waiver program in the following ways 1) you do not have to meet the financial eligibility requirement for Medical Assistance and 2) you do not need to meet the Nursing Facility Level of Care that is required for the Attendant Care Waiver program.
Attendant Care Waiver
The Attendant Care Waiver Program is funded through the Commonwealth of Pennsylvania, Department of Public Welfare. The program helps adults with physical disabilities improve their quality of life. The goal is to increase their independence, enable them to remain in their home, and seek or maintain employment.
The Autism Waiver, funded by the Commonwealth of Pennsylvania, Bureau of Autism Services is designed for adults, age 21 and older, diagnosed with an Autism Spectrum Disorder. The goal is to improve their quality of life by enabling them to live independently in the community. Additionally, the Waiver is intended to help them seek and maintain employment.
The COMMCARE Waiver, funded by the Commonwealth of Pennsylvania, Department of Public Welfare, provides services to eligible persons with a diagnosis of traumatic brain injury in order preventing institutionalization and allowing them to remain as independent as possible in their own home.
Community of Choice Housing Grant
This is a service made possible by a grant from the Pennsylvania Developmental Disabilities Council. The goal of the Community of Choice Housing Grant is to provide young people assistance in developing a support network to help them examine where they would like to live. The network, which is called “Circles of Support” creates goal-oriented and person-centered plans.
Financial Assistance for Home Modifications
Financial assistance isn’t just for enhancing independence through personal care and support services. Many UDS clients with permanent disabilities and modest incomes are able to receive financial assistance for needed home modifications through local, state and federal programs. Whether you rent or own your home, we can help you determine if you qualify for government assistance.
The Independence Waiver, funded by the Commonwealth of Pennsylvania Department of Public Welfare, was created to serve people with physical disabilities who are eligible to receive nursing facility care in their own home. The state allows that federal funds normally used to pay for individual’s care in a nursing home can be used to care for them in their home or communities.
The OBRA Waiver is a Home and Community Based program funded by the Commonwealth of Pennsylvania Department of Public Welfare (DPW). The program offers support to adults with developmental or physical disabilities whose disability occurred before the age of 22. The goal is to improve quality of life by enabling you to live independently in your own home, thereby preventing inappropriate institutionalization. UDS is a Supports Coordination agency and provider for personal care and personal care attendant payroll services.
